All office hours with The Eyewerks Optical Shoppe and Dr. Collazo
are by convenience of appointment to accommodate our patients'
busy lifestyles. We are available days and evenings for appointments,
including Saturdays. Gift Certificates are available. Don't forget
that you can use your Flex Spend Dollars.

Fees and Insurance
In order to keep our fees affordable, we request full payment
on the day of your visit to our office.
For the convenience of our patients, we put together a superbill
for you to present to your insurance company for reimbursement
Out of the many insurance plans, we have selected only those plans
that allow us to deliver the highest level of care to our patients.
These include Aetna, Blue Cross/Blue Shield PPO and Traditional
Plans, Cigna PPO, VSP. Of course, Medicare is accepted.

Cash, personal checks, Visa and Mastercard are accepted.
